Registrar student information change form

To change information such as name, social security number, date of birth, address, phone, or email; you will need to submit a student information change form to the registrar along with supporting documents. The information will then be updated.

Change in Name

Students with changes in name should complete a student change form. Students must present, to the Enrollment Support Center, photo identification with legal verification of change and one of the following: social security card, marriage license, divorce decree, legal court order. Only original documents will be accepted.

Change in Address, Email, or Phone

Changes in address and/or phone may be updated on BannerWeb through the student portal. Students may update changes in address, email, or phone numbers by completing a student change form. Students must present photo identification in the Enrollment Support Center or use secure student email. Electronic requests, via secure student email, must include the student’s ID number and be emailed to RegistrarsOffice@GwinnettTech.edu


If you have had a name change, you will also be given a new email address.
